Some sections ask you to give us more information. Where they do, please make sure that you give us as much information as possible. This will help us to award you as many points as we can.

POINTS

We will award points to you if:

• you are homeless (we will ask you to provide information that shows you are homeless)


• you are currently sharing facilities


• you are living in poor housing conditions


• you are living in overcrowded conditions


• there are medical factors that make your current home unsuitable


• you have specific personal needs that are not met by your current home

We may visit you at home to check all the information you put on your form.

ABOUT YOUR HEALTH

If you tell us that your, or a member of your family's, health is made worse by your home, we need to know about the medical condition. You can do this by:

• sending us a photocopy of your current prescription


• bringing in your medication for us to photocopy the bottles/packets


• sending us photocopies of your sick notes from your doctor


• tell us on your form how your health is affected by your current home

We may need to telephone you to ask you questions about your medical condition. Please tell us if we cannot phone you during the day.

IF YOU OR YOUR PARTNER IS PREGNANT

If you or your partner is pregnant we must have a photocopy of your 'Certificate of Expected Date of Confinement' (this is a form that your doctor will complete for you; it tells you when your baby is likely to be born).

YOUR IMMIGRATION STATUS If you are not a British Citizen, we need information on your immigration status issued by the Home Office to process your application.
